Dear Bachelor & Foundation students (Year 2020, 2021 & 2022 Intakes),

Please be informed that students who have NOT submitted the following documents to the University:

a) Original hardcopies of the Health Examination Report, Declaration form, Withdrawal from Studies and Fees Refund Policy form (if you have misplaced these
forms, you can download it via https://admission.utar.edu.my/ - please scroll towards the bottom),

and

b) Educational certificates (as submitted with the application form), IC (Malaysian students)/ Passport (International students) and other supporting documents
(if any) for verification.

You are requested to submit the above documents when you are on campus at the Division of Admissions and Credit Evaluation (DACE) at Heritage Hall, Ground Floor (A006), UTAR Kampar campus.
